This is the 99100613 Snoway Mount.  This is for a 29 or a 32 series plow only. 

It is a 2-pin mount  for a 2003-06 Chevy 2500 Silverado 4x4, 2003-06 GM 2500 Sierra, 2003-06 Chevrolet K3500 pickup, and a 2003-06 GM K3500 pickup.
